Scope of Work – Repair & Troubleshooting
1. Site investigation and troubleshooting of fiber infrastructure along S. Matlock Rd south of Green Oaks Blvd. 2. Locate and expose damaged or compromised conduit sections as identified on attached map. 3. Perform necessary repairs to restore continuous fiber pathway (conduit repair, splicing, or re-routing as required). 4. Test and verify fiber continuity and signal integrity after repairs are completed. 5. Document all findings, repairs performed, and provide as-built updates. 6. Coordinate with City of Arlington representatives for inspection and approval of completed work.
Notes & Assumptions
• Work to be performed during normal business hours unless otherwise requested by the City. • Traffic control and permitting (if required) to be coordinated with City of Arlington. • Any additional damage or scope discovered during investigation will be documented and quoted separately. • All work will be performed in accordance with City of Arlington specifications and industry standards.

• Clear physical damage confirmed on both Fiber #7 and #8 at approximately 1,643 ft from the test location. • Fiber #8 shows 0.772 dB loss event + high attenuation section (0.700 dB/km) 38 ft past the enclosure. • Damage location matches reported third-party impact (AT&T contractor bored through conduit). • Fiber 7 was broken inside splice enclosure in order to isolate damage and confirm damage location.
